Scheduling Appointments: When you need an appointment, please call our office. Our number is (937) 431-3779. We will take your basic information. We will ask you the reason for your visit to ensure that enough time is allocated for your appointment.

 

Registration: Please plan to arrive for your appointment on time. You will be asked to present your insurance card and verify your address and phone number. If your insurance has changed since you last visit, please present it to the receptionist to make a copy for your chart.

 

Office Charges and Copays: Both our physicians participate in a large number of insurance plans including Medicare and Medicaid. However, would appreciate if you would contact your insurance to verify that the services will be covered. We collect your insurance co payment at the time of checking in. If you do not have insurance, your visit will be your responsibility, and you will be required to pay the office visit in full. For your convenience, we accept cash payments, personal checks, Visa and MasterCard. For all checks returned for “Non sufficient funds,” Integrated Medical Group will charge you an extra $25 as service charge.

 

After Hours Care: Our after hours number is (937) 463-2400. The answering machine will contact the doctor on call and the physician on call will contact you and advise accordingly.

 

Medications and Refills: When our physicians see you in the office during the appointment, they will generally write your prescription that you regularly take, with a certain number of refills. Follow up appointments are required to monitor your progress on these medications. If you have picked up your last refill at the pharmacy, it means it is time for you to call our office for an appointment.

 

Missed Appointment Policy: Integrated Medical Group requests you to call the office at least 24 hours in advance in case you need to reschedule or cancel an appointment. Our staffs do make courtesy calls on the eve of your appointment, confirming the time.

 

Billing office: If you have questions or concerns about your bill, please feel free to call our billing office at 1-888-288-5551. If you need further assistance, please do not hesitate to call our office.

 

Release/ Transfer of Medical Records: Integrate Medical Group charges a fee for copying and mailing your medical records. The medical records will be mailed directly to the physician/ facility you have designated in your authorized letter. Upon receipt of your payment, the copied records will be mailed. Integrated Medical Group’s fee policy is in accordance with the Ohio Revised Code 3701.741. Break down of the charges are as follows:

  • $15 labor charge
  • Pages 1-10: $1 a page
  • Pages 11-25: .50 cents a page
  • Pages 26-51: .25 cents a page
  • Only the last three years of your chart will be copied unless you specify otherwise
  • Under no circumstance, will the original medical chart be given or mailed
  • Please allow at least 10 days time for your request to be processes.
